Agreement for bookings

Thank you for booking an appointment with Dr Tracie O’Keefe DCH, BHSc, ND

To confirm your booking, please read the terms and conditions below, enter your contact details, date and appointment details, payment details, and submit your agreement.

TERMS OF PAYMENT

I agree to purchase a therapy session (total of 1 hour) with Dr Tracie O’Keefe DCH, at a total cost of $330 AUD.

I understand that:

Full payment for the session must be made in advance.

CANCELLATION POLICY

If you need to change or cancel your appointment you will need to make those alterations at least 7 DAYS in advance BY TELEPHONE ONLY: 0403 398 808.

Changes or cancellations made less 7 days in advance will incur the charge of the FULL rate for that session.

Missed appointments or late cancellations will be charged to your credit card, or your credit will be used up if you are in advance.

Payments are processed securely using Stripe.
